7. Work order authorization

Please read and accept to authorize us to begin your repair.

Authorization for Repairs. I hereby authorize The LAB Group Inc. to perform the repairs as detailed in the estimate provided by my insurance company. I further authorize the operation and/or transportation of the vehicle for the purpose of repair. I understand this estimate may change due to manufacturer pricing changes and hidden damage discovered during repair.

Liability, Storage & Parts. The LAB Group Inc. shall not be liable for damage, theft, or loss of loose articles left in the vehicle. Storage charges at the municipality-authorized rate apply if the vehicle is not removed within 24 hours after notice of completion. The LAB Group Inc. may retain the vehicle until payment is made in full. Parts are assumed new and not OEM unless noted; if original parts are unavailable, aftermarket or used parts are authorized.

Early Release & Cancellation Fees. If release is demanded before completion: up to $2,500 + tax for administrative/processing costs; up to $500 + tax for removal of parts and re-assembly for inspection/repair; $100 + tax per day storage (Oakville) from arrival to removal; and a $150 + tax non-drivable removal fee.

Payment & Insurance Assignment. I authorize interest of 2% per month (24% per year) on outstanding amounts. I irrevocably assign to The LAB Group Inc. my insurance proceeds sufficient to pay for the repairs and direct the insurer to pay The LAB Group Inc. directly, who may endorse/sign co-payable cheques on my behalf and deposit them. All statements made by me are true; if coverage is denied for any reason including misrepresentation, fraud, or improper disclosure, all balances will be paid by me in full.
